Provides support to a number of individuals who live in their own homes. 24 hour support is available although it is planned around the needs of the individual offering as many hours of support as required. Provides a range of services to help people with disabilities to access and make full use of mainstream facilities. Provides information and support in leisure, education, training, voluntary work and supported employment. Referrals can come from local councils, social work, care managers and local area co-ordinators. Self referrals also considered.
