The service is a transition service that is staffed 24 hours a day and specialises in supporting younger adults with daily living skills and emotional support, developing individuals to move on to more independent living. Specialist Expertise: This service specialises in supporting people who have come from residential settings and enables the individuals to have a smooth transition from residential to supported living. They ensure enablement and inclusion at all times for those people they support. By doing so, they enable individuals to have new experiences, they are supported to make lifestyle changes based on their choices and decisions which previously others have made for them. Staff actively support and encourage individuals to attend college, find employment both paid and voluntary and participate in the local community.
