Outreach is a service that enables adults within the autistic spectrum including conditions such as Asperger syndrome and P.D.A (Pathological Demand Avoidance), to receive qualitative support in the places they live, work and relax. The aim is to support these individuals in developing the living skills needed to make the transition from residential homes to independent or supported living within the local community. In addition, Outreach provides much needed assistance to the families and carers as well as support to individuals living in their own home. All referrals to the service are deemed important. Following a referral an assessment is undertaken to establish the needs and levels of support necessary to provide a qualitative service to each individual. Outreach is underpinned by the outcomes of: maintaining and developing relation/friendships; informed/supported choice; a role in the community; being safe; having a means to communicate and being communicated with; respect and trust; being person led/interest led; staying healthy.
