Located in the Farcet area of Peterborough, Conquest House is able to provide support to adults with autism, learning disabilities, mental health conditions, and behaviour that may challenge. Our team is passionate about helping individuals to lead lives that are as independent as possible. Our approach to support: The support at Conquest House is delivered by a professional multi-disciplinary team committed to providing care focused on the needs of the individual. We believe in a personalised approach and everyone living at Conquest House has their own person-centred plan which is reviewed regularly. Our aim is to provide support enabling individuals to build on skills, live more independently, and reach their goals. We also believe that individual choice is paramount and each person we support is encouraged to make informed choices and maintain independence. A key worker is assigned to each person ensuring a holistic approach to individual care. We encourage individuals and families to actively participate in the planning of support, which is facilitated through individual/relatives meeting where information is provided. Conquest House work closely with specialist local services such as: The community learning disability team; Community nurses; Psychologists; Speech therapists; Local health centres; Dentists; and Chiropodist.
