Crews is a new service set up to support children, young people and adults diagnosed with a learning disability, dual diagnosis and individual who have other health support needs. Our expert area is autism among other diagnoses. We accept referrals from a variety of routes including the health & social care sector and private individuals. Crews prides itself on the delivery of person centred and dignified support and on receipt of a referral, the dedicated managers will visit the individual or family to undertake an assessment of needs. This is used in additional to other valuable information provided by the individual, family members and professionals where appropriate and used to develop a person centred care and outcomes plan. The plan is recorded in one Statement Format for use by the allocated front line staff. Workers are matched to service users determined by experience, training and personality. We provide support in a variety of settings including the individual's home, the community or educational settings for example. We take a task centred and motivational approach to our work to increase levels of independence and enhance quality of life. The service is set up and run by managers who have worked in the sector for many years and are qualified social workers and care managers. The service caters for ages 0 to 100 years.
