Hazeldene
Main information
Type of service: Residential services
Service URL: http://www.choicecaregoup.com
Hazeldene is a large, detached house - converted from two semi-detached houses - situated just to the west of Reading town centre providing support for six adults, who are more able, with mild to moderate learning disabilities and who have associated complex needs. The home has six single bedrooms, all decorated to the service users' individual tastes, two kitchens, two lounge areas, a dining room, a small quiet/music room and a large garden. There are separate areas within the home for educational and leisure time activities. However, the need for home-based activities is limited due to most of the service users living in the house attending external activities such as college, work placements, skills training and art & drama groups. In addition to Hazeldene is Wantage Road, a large, semi-detached house providing residential care for four adults of higher ability, with mild to moderate learning disabilities and who have associated personal and emotional difficulties. Although Wantage Road is next door to, and is registered with, Hazeldene, it is quite a separate home, offering individuals the opportunity to progess on to independant living. Referrals can be made by a Social Worker/Care Manager or directly from parents themselves. However, before assessment can take place the Care Manager acting on behalf of the service user must be consulted.
