Provides Domiciliary Care and Support including Personal Care, Supported Living, and a 5 bedded Residential Home, to adults with Learning Disabilities and/or Autism, Mental Health Needs including Asperger syndrome, individuals with a dual diagnosis and complex needs, and those with physical disabilities/sensory impairments. Specialises in providing positive behaviour management to individuals who require consistent and structured care and support by trained and skilled staff. All referrals are dealt with by their Referrals Manager (Lorraine Burton) who takes the initial details then organises visits to the individual/relatives/professionals to carry out a Care Needs Assessment. Once this is established their PCP manager collates information with the individual and all stakeholders to produce comprehensive person centred plans which include life history, aim and ambitions, support plans and daily routines, Health Action Plan, Communication Passport, Timetable of Activities. Accepts Adults Social Care and Personal Budget referrals.
