The service is able to support people with learning disabilities, mental health and/or substance misuse issues to develop independent living skills. We also accept emergency placements.
The service is a new four-bed shared, 24-hour supported living service. Individuals hold tenancy with a separate housing provider. Service users have individual rooms situated on the first floor with shared use of the ground floor communal lounge, kitchen/diner and utility area. In addition, there is a self-contained gardeThis service would be suitable for anyone moving from residential care or their family home who needs a period of transition before moving to more independent living environments. The service provides 24-hour background core support, including a sleep-in night member of staff. Following unified assessment an individual support package is provided and 1:1 hours detailed in line with identified need. .
The service aims to support people to live a full and active life, and to achieve greater independence, either by a reduction of 1:1 hours or supporting individuals to move to their own accommodation with outreach support, if required, to provide continuity of support.
