Our Community Support Service Team is experienced in providing high quality support and personal care to people of all ages. With a person centred approach, they create a team that works with you to formulate a plan to enable them to achieve their goals and maximise their potential. We specialise in the delivery of support and care for people of all ages who have: learning disabilities; autism and sensory impairments; physical disabilities and complex health issues; acquired brain injuries or associated mental health issues; challenging behaviour and communication issues. The type of support we can provide is: transitional support as you leave education or move into less supported housing; support to access education, work, health or other universal services; assistance to pursue leisure and social activities; help with day to days tasks in your own home or local community; and development of independent living skills. We look after your best interests and alleviate the anxieties of life's problems in a realistic, but positive way. Some of the barriers we can help you to overcome are: managing your relationship with your landlord/housemates; meeting and making friends; maximising your benefits and exploring opportunities to earn money; furnishing your home and managing your income; and, resolving Appointee, Guarantor and funding issues.
