The Light House is a multi-disciplinary, multi-agency service aiming to meet some of the needs of disabled children and their families.The team work with children and their families in Derby and some children and families in the south of Derbyshire, aged 0-17 years. It can provide information, advice, sign posting to other services and a route to assessment and if needed, services for disabled children and young people and their carers. The Light House provides, with partners, an Holistic Assessment Nursery which enables for the education, care and development needs of disabled babies and toddlers. The Residential Short Break Service provides for children across South Derbyshire who have severe challenging behaviour or who are Profound and Multiple Learning Disabled aged 8 and above (or from the age of 5 in exceptional circumstances). Children with a severe learning disability who have social care needs living in Derby may also receive a residential overnight service. To access the service a common assessment must be undertaken. This must be completed by a professional already working with the child, that knows the child and the perceived difficulties, e.g. school teacher, community nurse, health visitor, paediatrician, occupational therapist or physiotherapist that will identify any unmet needs.
