Offers a broad based service to children with severe learning disabilities who present with challenging behaviours and children with moderate to severe learning disabilities who have health needs. The team provides a service to the child, their family and/or carers. Children can be supported in their home environment, local community, school or in respite care/residential settings. The team offers assessment, Health Action Plans, support, advice, practical assistance and transitional support towards adult services. The service operates an open referral system.
