The transition support helpline/case work service provides advice and support to young autistic people and their families on making the transition from school, further or higher education to adult life.

Call 0808 800 0027 - Leave a message on our answering service and a volunteer adviser will call back (free from landlines and most mobiles). We are a small team and a very busy service so it may take several days for a volunteer adviser to get back to you. Alternatively, you can contact us by filling out our online enquiry form below.

- offer information, advice and support by phone or email
- explain rights and entitlements in relation to transition planning, including education, social security benefits and community care in England, Scotland, Wales and Northern Ireland
- provide information on requesting assessments and accessing support within the community
- help young autistic people and their families explore their options and make informed decisions
- provide guidance and support on specific issues such as engaging young people, finding suitable provision or resolving disagreements.

- provide face-to-face support or attend meetings, but we can help you prepare for them
- contact professionals or write to them on your behalf, but we can write letters with you
- suggest appropriate provision or professionals but the Autism Services Directory may help
- offer legal support but we can signpost to relevant organisations.
