Brookdale Care is an independent provider of hospital, residential, respite (short breaks) and supported living care for people with autism spectrum conditions (ASCs). Brookdale Care focuses on meeting the challenging and complex needs of adults with autism and Asperger syndrome. We believe that individuals with ASC are unique in their needs and abilities and our approach involves providing a structured therapeutic environment that centres on personalised care pathways to a more independent style of living. Our aims: We work with the individual, staff, families and commissioners to provide bespoke care and treatment plans that enable our clients to achieve positive outcomes. We achieve these by: Ensuring that the client's needs and wishes are at the centre of their care plan; Providing a safe, secure and nurturing environment to help clients to develop independent living and social skills; Clinical and therapeutic teams work holistically to provide robust support and intervention; Ensuring our staff receive training and support to give them the right skills to meet clients' individual needs; Supporting clients to attend college and further their education; Enabling clients to develop the skills to gain and keep employment; and Promoting a healthy physical and mental environment that contribute to clients' independence, fulfilment and overall well-being. Positive Outcomes: We work closely with Learning Disability, Forensic and Mental Health Commissioners and social care staff to provide tailored care pathways, which enable individuals to achieve positive outcomes including: Well-maintained health and physical well-being; Improved emotional and mental well-being; Improved social skills; Better independent living skills; Increased personal choice and control; Enhanced personal dignity; Reduction in risk of harm to self or others; and Transition to more independent living..
